I adopt the recommendation made by the State Coroner in previous inquest involving a death resulting from contact between police and the public, (the most recent being Skews and Crome 1304 and 1305/94) of the need for ‘Critical Incident Review’ of each incident. This is essential in order to assist the Police Force to identify potential areas for improvement and for the coronial investigation process.
